Description

Job Title: Nurse Practitioner/Physician Assistant

Location: Clayton County, GA

Job Status: Per Diem with no guaranteed hours

Position Summary: Perform in-person, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members of specific health plans. These assessments will take place within the member’s personal living space according to market health plan expectations.

Hours: Flexible scheduling M-F for 55-65-minute appointments during days and evenings.

Position Essential Functions

  • Complete required training within the specified time.
  • Provide a minimum of 25-30 appointment slots per week.
  • Perform high-quality assessments to meet member satisfaction and health plan standards.
  • Document and submit chart notes within 24 hours of the visit.
  • Address charting queries within 48 hours.
  • Maintain communication with clinical leadership staff.
  • Attend training sessions and meetings as required.
  • Enter personal living spaces for scheduled assessments.
  • Lift up to 20 lbs.
  • Bend or kneel for extended periods if needed during assessments.

Position Requirements

  • Strong computer skills and proficiency with electronic medical records.
  • Understanding of risk adjustment models for CMS and ACA.
  • Accurate and thorough documentation of assessment findings.
  • Effective verbal and written communication skills.
  • Maintain confidentiality in compliance with HIPAA regulations.
  • Make critical decisions following health policies and procedures.
  • Adapt easily to changing conditions and responsibilities.
  • Display a positive attitude and maintain collaborative relationships.
  • Proactively address and solve member, provider, and health plan issues.

Education And Experience

  • Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP).
  • Master’s degree from an accredited PA program.
  • Preferred: Home health experience and 1 year of in-home assessment experience.

Certification And Licensure

  • Current RN and NP or PA licensure in the state of practice.
  • Board certification by ANCC, AANP, or NCCPA.
  • Current Medicaid ID number for the state.
  • Current Medicare PTAN Number.
  • Current BLS certification.
  • Valid state driver’s license.
  • Valid proof of sufficient car insurance coverage.
